Chelsi Frazer
Chelsi Frazer, MSN, APRN, FNP-C, is a Family Nurse Practitioner with more than eight years of nursing experience in outpatient medicine and the neuro-intensive care unit. Her experience in critical care has provided a strong clinical foundation in evaluating and managing a wide range of acute and chronic medical conditions.
After working as a registered nurse, Chelsi earned her Family Nurse Practitioner degree to expand her role in diagnosing, treating, and managing patients across the lifespan. She provides comprehensive primary care, including preventive care, annual wellness exams, treatment of acute illnesses, management of chronic conditions, and routine health screenings.
Chelsi values taking the time to understand each patient's health concerns and developing treatment plans that are practical, evidence-based, and tailored to their individual needs. She believes that educating patients about their health allows them to make informed decisions and play an active role in their care.
She is currently completing her certification in Functional Medicine through The Institute for Functional Medicine (IFM), where she is expanding her knowledge of evidence-based approaches that complement traditional primary care by addressing lifestyle, nutrition, and other factors that influence overall health.
Chelsi is committed to continuing her education and staying current with advances in primary care to provide patients with comprehensive, up-to-date medical care.
